This form will serve as your formal request for changes, updates or other requests relating to your website. Once we receive your work order, we will get back to you as soon as possible with an estimate of costs. After the cost estimate is issued, we will require written approval from you via email, fax or U.S. mail before we proceed. Upon written approval, a project timeline will be generated and an estimated date of completion will be provided to you.

Your business is important to us and we want to continue to provide you with the high level of customer service to which you are accustomed. Our goal is to ensure that your requests are completed in a prompt, concise manner and that your costs are clearly outlined at the start of every project.

Our rate schedule is as follows:
HTML / Web Development: $85.00/hr
Programming / Database Development: $110.00/hr
Rate discounts, if applicable, will be included in your quote for services.

All items are required.

*Rush Requests: Rush requests will be handled as quickly as possible. In most cases, if you submit the request early enough in the day, your turn around time will be the same business day, however, this may not always be the case. If we are not able to complete the work by the following business day, you will not be charged the rush surcharge. It is not always possible for all requests to be handled as a rush (depending on the extent of your programming request and the time of day that you make the request). We will advise you of any delays and will confirm whether a rush timeframe can be met.
